Requirements
The following requirements must be submitted to the Admissions Office prior to enrollment in any
academic program:
Grade 11
● Original Form 138/SF9-JHS (Learner’s Progress Report Card)
● Original Form 137/SF10-JHS (Learner’s Permanent Academic Record)
● Colored Photocopy of PSA-issued Birth Certificate
● Original Copy of Certificate of Good Moral Character
● 2 pieces 2×2 ID picture
Freshmen
● Original Form 138/SF9-JHS (Learner’s Progress Report Card)
● Original Form 137/SF10-JHS (Learner’s Permanent Academic Record)

Transferees

Qualifications of MBA
● Applicants must be graduates from business-related courses.
● For non-business graduates, students are required to complete 9 units of bridging
subjects, which may include Basic Accounting, Finance I, and Principles of
Marketing.
Qualifications of DBA
● Applicants must hold a master’s degree in business.
● For those with a non-business master’s degree that includes a thesis, the applicant
must complete at least 9 units of cognate subjects.
● For holders of a non-thesis master’s degree, students must take a 6-unit thesis
writing course before enrolling in the regular curriculum.
Modes of Application and Enrollment for School Year 2025–2026
1. Visit apply.psba.edu and select “New.” Complete the application form with accurate information.
2. Pay the reservation fee* at the Campus Cashier.
3. Prepare the necessary admission requirements for submission on or before the start of classes.
*Note: Paid reservation fees will be deducted from the total assessment fees.
1. Visit your preferred PSBA campus during office hours.
2. Fill out the application form provided at the Admissions Office.
3. Pay the required amount indicated on the assessment form or the reservation fee at the Cashier’s
window and secure the official receipt.
4. Prepare the necessary admission requirements for submission on or before the start of classes.
